błąd podczas rejestracji

Witam! Klikam na zarejestruj się, potem na akceptuję warunki, pojawia się strona z formularzem lecz na górze strony, nad nagłówkiem pojawia się coś takiego:
linia 4548-4552 a tego pliku wygląda tak:
wczoraj dodałem mod captcha_simplemath.
co to może być?? ostatnio ludzie się rejestrowali na forum, więc to chyba nie przeszkadza lecz błędów wole nie mieć ;/
proszę o pomoc
